HEALTH GUARANTEE

 It is our hope that you will follow the advice and instruction of your veterinarian. By adoption date, I will have supplied you with a health record of the puppy you have adopted. I suggest that you please schedule a well-baby appointment with your veterinarian so that your puppy starts off on the right "paw".

 A deposit of _____________ will hold your puppy until it is ready for its new home. The remainder is due at time of pick up.

All deposits are non-refundable except as listed below.

 Your puppy received his/her first immunizations on ___/___/___, administered sub-q by _________ ____________________ ( veterinarian); see copy of health record. The 2nd set of immunizations will be due 3 to 4 weeks from 1st shot. It is recommended that you follow your veterinarian's shot schedule beyond the second set of immunizations. Your veterinarian should schedule your appointments appropriately to make sure that your puppy is protected against life-threatening diseases. A Rabies vaccine should be administered by age 4 months (___/___/___). At that time, your puppy may need a kennel cough or perhaps distemper/parvo booster.

 We strongly advise our adoptive families to follow the advice and instruction of your veterinarian. DO NOT EXPOSE YOUR PUPPY TO OTHER ANIMALS and/or potentially dangerous environments until he/she has had ALL puppy immunizations and your veterinarian states that your puppy is safe to be exposed to such conditions.

 Three Month Guarantee: A veterinarian's health examination has been performed on this dog. This dog has been pronounced to be of sound health free from any congenital birth defects. There is a three month guarantee on this dog for these types of defects provided by the Seller. This guarantee is contingent on a written veterinarian's statement noting the types of tests that were administered to determine that the defects were present at birth and an examination by the Sellers veterinarian concludes the same diagnosis. At that time, this dog can be returned to the Seller for another puppy. The Seller assumes no responsibility for any veterinary bills as a result of this other than the Seller's own veterinary bill for the confirming examination.

 72 Hour Guarantee: On Adoption Day, your puppy is HEALTHY! You have 72 hours (including adoption day) to have your puppy examined by your veterinarian of choice. If your puppy is diagnosed with a heart, lung, liver or bone malfunction within the first 72 hours of adoption, your adoption fee will be applied to another puppy. Your puppy must be described in detail (sex, color/markings, etc.) on the vet's documentation of anything found wrong with the puppy. Before your puppy is replaced, I will take puppy in question to my veterinarian as well for a thorough exam. This health guarantee does NOT cover intestinal worms or parasites, mites of any kind, eye or skin-related conditions. We will NOT refund veterinary expenses. Any travel/shipping expenses are YOUR responsibility. This Health guarantee is contingent upon the New Owners commitment to have the puppy examined by a licensed veterinarian within 72 hours (including the day of adoption), otherwise the New Owner forfeits ALL rights.

 Bulldog breeds are prone to certain health issues unique to the breed. You should research the breed before you decide to purchase. 

It is also understood that new owners through IOEBA and WBA will register the above dog/puppy with ___________of SFB on its registered name.

If buyer cannot keep above dog/puppy at any point, seller must be notified ahead of time and given first option of taking back the dog/puppy at no cost to seller.  If it is found that above dog has been placed without notification, surrendered or sold to a pet store, wholesale outlet, dog pound/shelter, or rescue organization, Seller reserves the right to take immediate repossession of dog and fine buyer $2,000 (two thousand) dollars. 

Any returned animal must be accompanied by any and all paperwork supplied by the breeder at the time of sale, as well as a written statement from a licensed veterinarian indicating the following: a) all shots are and have been kept current b) dog/puppy has had a negative fecal within thirty days c) dog/puppy has had a negative heartworm test within thirty days and has been kept on heartworm preventative d) dog/puppy appears to be healthy and well cared for.

Our Miniature Poodle Contract

STELLA’S FABULOUS BULLDOG CONTRACT FOR POODLES

SFB

 

The seller,__________________________________, and the buyer,__________________________, hereby agree as follows:

 

1:  The seller hereby sells to the buyer, for the amount of $__________. Payment of which is hereby acknowledged, a male/female Miniature/Toy Poodle.  The name of the Poodle must have SFB’s at the beginning of the name, for that is the name of my kennel.

2:  The seller guarantees that the puppy is in good health on the date of delivery, has had first vaccine, worming and a vet check.

3:  The seller guarantees that the puppy is free of hereditary defects to the best of her knowledge at time of sale.  Should a puppy be diagnosed with a hereditary defect within 72 hours of purchase, the puppy MUST be returned for a replacement when one comes available.  The seller does not assume any additional liability for possible defects after 72 hours and will under no circumstance be responsible for veterinary bills or shipping resulting from any condition.

4:  The buyer shall maintain the puppy in good healthy condition, including adherence to a regular yearly exam and all vaccines as set to their vets schedule.  Be on heartworm preventative, good diet, and a clean safe environment.

 5:  If at any time in its life the buyer can not keep puppy, it must be returned to seller and can not be sold or given away unless it is approved by the breeder.

6:  The seller does not guarantee size or color as Poodles can change colors up to age 18 mos to 2 years of age.
